This Case Spotlight highlights the use of laser hair removal (LHR) with the 1064 nm wavelength on the Gentle system to prevent the recurrence of pilonidal cysts. Pilonidal cysts, often caused by ingrown hairs, can lead to painful abscesses, if untreated. The case study involves a 20-year-old male with Fitzpatrick Skin Type V, who experienced a recurrent pilonidal abscess post-surgery. He underwent 10 LHR sessions, using the GentleYAG® system to reduce ingrown hairs in the affected area.

The treatment included pre-session shaving, cleansing, and topical anesthetic application, followed by laser treatment with specific parameters (spot size: 15 mm; fluence: 14–24 J/cm²; pulse duration: 3, 5 or 10 ms, and DCD epidermal cooling). Post-treatment care involved soothing creams and moisturizers. Results showed a significant reduction in ingrown hairs and no recurrence of the cyst after the second session. The patient reported an improved quality of life and high satisfaction.

LHR is an effective preventive approach for pilonidal cyst recurrence, particularly for darker skin types, due to the deeper penetration of the 1064 nm wavelength and the advanced cooling system of the Gentle laser. It minimizes discomfort and risks like post-inflammatory hyperpigmentation, making it a reliable long-term solution.
